40 hours / week: Monday - Friday 8:00am - 4:30pm

About the Job: The RN Triage position in the Hospice Department is a critical, Non-Exempt clinical communications role updated for September 2026. Based in southern New Hampshire, this professional serves as the central bridge connecting patients, families, remote field staff, and physicians. This role primarily manages high-volume call prioritization and care coordination from the office, while retaining the operational flexibility to step into the field to deliver hands-on clinical visits, admissions, and death pronouncements when necessary.

What You'll Do:

• Manage & Triage Calls: Field and prioritize urgent patient and caregiver calls, providing direct clinical support, guidance, and assessment.
• Coordinate Interdisciplinary Care: Act as a central liaison between families, physicians, and intake departments to streamline patient orders, prescriptions, scheduling, and referrals.
• Support Field Staff: Serve as an accessible, real-time clinical resource for remote field nurses via phone, in-person meetings, or secure messaging.
• Document in Real Time: Maintain meticulous, immediate medical records of all calls, actions, and triage advice directly within Epic.
• Provide Field & Leadership Backup: Conduct scheduled or emergent field visits (including routine care, admissions, or pronouncements) and provide coverage for Clinical or House Managers as needed.

Who You Are:

• A Licensed Nursing Professional: Graduate of an accredited school of nursing with an active RN License from the New Hampshire Board of Nursing (or active compact state licensure) and current CPR Certification.
• An Experienced Caregiver: Possess at least 3 years of recent acute care experience in a community setting (5 years preferred), ideally combined with a background in community health or end-of-life care.
• An Expert in Palliative Care: Equipped with comprehensive knowledge of pain/symptom management and terminal illness care, ideally backed by a BSN or MSN and knowledge of Medicare Conditions of Participation (CoPs).
• A Skilled Communicator & Tech-Savvy Adaptor: Proficient with Epic EMR software, possessing excellent multi-tasking, problem-solving, and time-management skills under pressure.
• Vetted & Mobile: Ready to clear background compliance with a valid driver's license, satisfactory criminal record, and Bureau of Aging and Adult Services (BAAS) check.
