The Nel Permanent Supportive Housing Case Manager

Schedule: Sunday - Wednesday | 8:00 a.m. to 6:30 p.m. (30-minute lunch break)

Laurel Hill Center is seeking a Supportive Housing Services (SHS) Case Manager to support residents living at The Nel , a Permanent Supportive Housing community. In this role, you'll work alongside participants, property management, healthcare providers, and community partners to help individuals with disabilities achieve long-term housing stability and greater independence.

What You'll Do

As an SHS Case Manager, you will provide individualized support to residents as they navigate the opportunities and challenges of independent living. Your work will include:

• Building trusting relationships through proactive outreach and engagement
• Supporting residents in maintaining permanent housing and avoiding eviction
• Serving as a liaison between tenants, property management, treatment providers, and community partners
• Developing and updating individualized housing stability plans and other service plans
• Coaching residents in daily living skills, including medication management, budgeting, financial literacy, and self-sufficiency
• Responding to crises and helping participants navigate difficult situations before they escalate
• Connecting residents with community resources, benefits, healthcare, employment supports, and other services
• Providing transportation and community-based support as needed
• Maintaining accurate and timely documentation in an electronic health record system
• Collaborating closely with internal staff and external service providers to support participant success

Additional Requirements

• Ability to maintain confidentiality and comply with HIPAA requirements
• Valid driver's license and ability to provide transportation as needed
• Ability to perform the physical requirements of the position, including local travel, lifting up to 25 pounds, extended computer use, and a combination of sitting, standing, bending, reaching, and walking

Minimum Requirements:

Education and experience – must have at least one of the following:

• Lived experience of homelessness, mental health, and/or substance use plus a combination of 2 years of related employment, education, or volunteer work
• BA/BS degree in mental health, social services, or related field, or equivalent, plus one year of experience relating to or assisting individuals with mental illness, housing instability, and/or substance use concerns; OR
• Eligible to be certified as a Qualified Mental Health Associate through Mental Health and Addiction Certification Board of Oregon or meet the criteria to be eligible for certification after one year; OR
• Valid Oregon Driver License in accordance with Laurel Hill Center vehicle use policy and procedures
• Documentation of successful completion of a background and fingerprint check as specified by Oregon law, as well as a urinalysis drug screen (Although recreational marijuana is legal in Oregon, our policy—aligned with both federal and state regulations—requires all applicants to test negative for THC prior to employment)
