Clinical Pharmacist - per diem

Position Summary Assures the timely and accurate administration of medications and IV solutions to patients pursuant to therapeutically valid prescribed orders and in accordance with all state and federal regulations as well as established peer standards, hospital policies, procedures and protocols. Working with the Pharmacy Manager to manage the daily operations of all areas of drug distribution, supervises Pharmacy Technicians and Pharmacy Externs/Interns; follows pharmacy policies, procedures and protocols; participates in Performance Improvement activities and maintains readiness for JCAHO accreditation. Responsibilities Clinically responsible for evaluating the therapeutic rationale of all medication orders, providing information/education as needed, and for producing accurate medication administration records. Accurately follows various Clinical Pharmacy Protocols including High Dose Amino glycosides, Weight Based Heparin, Parenteral Nutrition, Adverse Drug Reaction Report, Renal Dosing. Verifies all aspects of medication orders and follows AMAR policy for Medication Process Performance Improvement. In response to new information or specific requests, educates medical, nursing, and pharmacy staff in a timely and accurate manner. Distributive responsibility for accurate and timely administration of medications and IV solutions. Monitors efficient receipt of physician’s orders, processing of orders and delivery of medications. Checks all medications for hourly distribution rounds. Completes Performance Improvement monitoring for Parenteral Nutrition. Loads, unloads medications, troubleshoots common issues such as drawer failure, controlled substance discrepancy, Pyxis/Meditech problems, and reviews “meds ordered not loaded” reports for prompt attention. Monitors correct computer entry for charging purposes and reviews entries for appropriate cost/charge. Obtains “short” inventory items or provides therapeutic alternatives, documents to assure product availability and appropriate management. Monitors inventory usage, rotation of stock and disposal of outdated, recalled or poorly labeled products. Works collaboratively with Pharmacy Technicians to ensure items are written on the list to be ordered from wholesalers. Provides supervisory/advisory responsibility to coordinate activities and promote teamwork within the Pharmacy and other hospital departments for optimal patient care. Completes mandatory housewide competencies. Assists in continuous training of pharmacy staff through observation and mentoring. Provides information and documentation pertaining to performance issues for all professional staff, including pharmacy, nursing, physician, and other professionals. Mentors new pharmacy staff, both pharmacists, technicians and students. Participates in Performance Improvement activities within the Pharmacy Department and interdisciplinary groups. Participates in assigned special department projects and committee work (Medical Staff Committees and Hospital Committees/Task Forces). Completes periodic competencies: both housewide and specific clinical competencies. Maintains professional competence through continuing education as required by the Commonwealth of Massachusetts and the Pharmacy Department. Recognizes safety hazards and takes corrective action whenever a hazard exists. Maintains personal safety by having a working knowledge of pharmacy and hospitalwide safety, disaster, and infection control plans. Serves as a role model and resource for other staff by sharing knowledge with colleagues. Seeks education and learning experiences in areas of identified growth needs and reviews progress during annual evaluation. Responsible for required documentation and maintains records as required by state and federal regulations and departmental policies, including controlled substances, dispensing records, and billing records. Completes the C‑2 Safe compare report each shift, resolves discrepancies, completes weekly C‑2 Safe, and monitors accuracy and documentation of controlled substances for all other areas of the hospital. Receives controlled substances from wholesalers and enters them into perpetual inventory. Completes and files DEA#222 form when controlled substances are received. Uses and documents refractometer readings for all controlled substances returned to the Pharmacy Department. Enters receipt and dispensing amounts of tax‑free alcohol from inventory, verifying account. Documents all loan/borrow transactions from other facilities in the Loan/Borrow Book and ensures timely return of borrowed items. Maintains combination control and restrictions regarding admittance to the pharmacy, observing non‑pharmacy personnel admitted. Requirements Bachelor's degree in science/pharmacy or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy with hospital experience including Unit Dose and IV Additives. Current Massachusetts pharmacy license or ability to obtain one by hire date. Basic Life Support (BLS) certification. Certificates, Licenses & Registrations Current Massachusetts Pharmacy License (must be obtained by hire date). BLS certificate. Knowledge, Skills & Abilities Computer literate with good interpersonal and written & verbal communication skills. Familiar with pertinent pharmacy, nursing service, and hospitalwide policies and procedures. Knowledge of Pyxis, Meditech, and Pyxis MedStation Pharmacy console. Knowledge of specific interactions affecting All‑in‑One TPNs. Familiar with policy & procedures for reporting loss of controlled substances, including completion of DEA form#106. Compensation $50.00 – $72.60 per hour (Boston‑area data; subject to modification based on geographic location).
