Job Title: Registered Nurse Profession: Nursing Specialty: Staff Augmentation Duration: 45 days Shift: Day Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of 2 years of recent RN experience working with individuals in home health, hospice, long-term care, or case management. License: Registered Nurse licensed to practice in the Client, active and in good standing without restrictions. Certifications: Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred. Must-Have: Excellent communication skills, including advanced abilities in explaining complex information to members and their support systems. Proficient with Google Office, Microsoft Office, and Adobe Acrobat. Description: We are seeking support from contract nurses to assist with rising nursing caseloads. Positions may require travel for in-person assessments. The role involves conducting nursing assessments and collaborating with external agencies. Assessments are used to authorize skilled nursing services for members. The successful candidate will conduct in-person visits and monitor health needs. The nurse will assist with referrals and advocate for members' rights. Documentation of medical necessity for skilled services must be completed. In-person assessments will be required every 90 days or as needed. The role is field-based and requires travel. The nurse will complete referrals and vendor calls as necessary. Participation in multidisciplinary planning meetings is expected. The nurse will collaborate with members and their support systems. Attendance at mandatory meetings and completion of annual trainings is required.
