Promise Point Care is seeking a compassionate and dedicated Case Worker to support and empower youth in a structured, trauma- informed environment. The Case Worker will play a vital role in helping youth develop life skills, achieve personal goals, and successfully transition into independent living.

• Manage a caseload of youth and provide ongoing support services
• Develop and implement individualized service/treatment plans
• Conduct assessments and monitor client progress regularly
• Advocate for youth needs and coordinate with external agencies (schools, counselors, courts, etc.)
• Maintain accurate and up-to-date case documentation and reports
• Provide guidance, mentorship, and crisis intervention when needed 
• Facilitate life skills development (education, employment readiness, coping skills)

Qualifications

• Experience in social work, human services, or working with youth
• Experience with at-risk youth preferred
• Strong communication and organizational skills
• Ability to build trust and maintain professional boundaries
• Compassionate, dependable, and team-oriented
• Ability to handle sensitive situations with professionalism
