Job Type Full-time Job Summary This position provides in‑home support to families, promotes a positive parent‑child relationship and interaction, and provides case management/linkage to community resources for parents/caregivers. The role advocates for families to receive their entitlements and benefits, housing connections, and more. Duties and Responsibilities Conduct field visits and escort families to various service providers, such as medical appointments, Public Assistance centers, Medicaid offices and housing services. Conduct home visits in compliance with ACS FTR requirements, assessing the needs of the family and child safety. Advocate and assist families with connecting to community‑based resources or organizations and making referrals to facilitate the services needed to the families. Provide supportive casework focusing on services goals, child welfare laws, improving family relationships, coping skills and parenting. Obtain collateral documents from medical, treatment and academic programs. Work with parents and provide education on improving parenting skills and home management skills. Participate in monthly Unit and Clinical Diagnostic Team Meetings. Complete and document case work activities in tracking logs, excel sheets, case record PROMIS, CONNECTIONS and other systems in a timely manner. Attend all required trainings to enhance skills working with a high‑risk population. Conduct case record/chart reviews on a monthly basis. Schedule and attend meetings and conferences as needed. Conduct community outreach and relationships with external providers. Conduct monthly chart reviews of at least 15 case records per month. Complete sections of the FASP assessment as required by ACS and OCFS. Meet required weekly face‑to‑face contacts with all parents and children assigned. Participate in school visits and IEP meetings. Monitor and obtain information on the child(ren)’s school placement and educational performance. Monitor and obtain information on the family’s medical needs, including compliance with periodic physicals and immunizations. Work with individuals, family and friend support networks, and health care and mental health professionals to put services in place. Requirements Ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful, caring manner. Demonstrates ability to organize and coordinate work within schedule constraints and handle emergent requirements in a timely manner. Ability to communicate effectively with supervisors, co‑workers, and other community partners. Ability to conduct interviews with families to obtain and provide information needed to carry out case management responsibilities. Knowledge and ability to use critical thinking and analytical skills to formulate with the family achievable plans for implementation. Ability to perform the essential functions of the job with or without a reasonable accommodation. Collaboration and facilitation skills to work with the individual, families and service providers. Demonstrates cultural sensitivity and competence in work with families from various ethnic and cultural groups, socioeconomic status and lifestyles. Qualifications HS Diploma or GED required. Basic computer literacy preferred. Due to the nature of the work environment, work hours may vary based on program needs. Perform other duties or responsibilities as assigned within the scope of the position. Successful completion of an Administration for Children’s Services‑funded prevention services program or similar program or life/lived experience with the child welfare system required.
