As a Social Services Assistant, you will play a vital role in enhancing the quality of life for our residents while supporting their social, emotional, and psychosocial needs. What You'll Do: Assist with resident assessments, care planning, and required documentation Support residents and families during admissions, transfers, and discharge planning Help residents adjust to long-term care and promote a positive living environment Advocate for residents' rights, dignity, and overall well-being Serve as a liaison between residents, families, and the interdisciplinary care team Assist with coordinating resident-centered programs and activities that promote emotional and social wellness Maintain accurate, timely, and confidential documentation Help connect residents and families with appropriate community resources as needed Support compliance with state, federal, and company regulations What We're Looking For: High school diploma or equivalent required Associate's or Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred Previous experience in skilled nursing, long-term care, healthcare, or social services preferred Excellent communication, organizational, and interpersonal skills Strong problem-solving abilities and attention to detail Compassionate, empathetic, and resident-focused mindset Ability to maintain confidentiality and professionalism in all interactions Why Join Gig Harbor Health & Rehabilitation?
