Compassionate Hospice RN Case Manager

Big Bend Hospice

Big Bend Hospice is seeking an RN Case Manager to provide compassionate end-of-life care in patients’ homes or facilities across the Franklin County service area. You will start with initial and ongoing assessments of physical, functional, psychological, and environmental needs, coordinating with the clinical team to deliver comprehensive care. The role requires an active Florida RN license, 2+ years nursing experience, EMR proficiency, and a willingness to travel. #J-18808-Ljbffr Big Bend Hospice
