Registered Nurse RN Home Health

Job Responsibilities Ensure all activities align with Elara Caring’s vision and the leadership of the Home Health team. Maintain patient and staff privacy and confidentiality in accordance with HIPAA Privacy Final Rule. Assess assigned caseload to identify physical, psychosocial, and environmental needs using documentation, clinical records, interdisciplinary reports, and on‑site evaluations. Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Collect data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Perform initial evaluation and re‑evaluate the patient’s nursing needs during each visit; make follow‑up visits as needed. Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care. Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members. Job Qualifications Current, unrestricted state RN license. 1+ year experience in a clinical care setting as a nurse. Ability to travel within branch/office coverage area, lift, push, pull, and support up to 50 pounds. Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Associate’s Degree or Bachelor’s Degree in Nursing (preferred). Previous Home Health or Hospice experience (preferred). Previous experience with HomeCare HomeBase (preferred). Benefits Work autonomy and flexible schedules. 1:1 patient care.
