After-Hours Hospice RN Case Manager

Gentiva is seeking an Afterhours RN Care Manager for our hospice team. In this field-based role, you’ll provide compassionate end-of-life nursing care during evening, night, and weekend hours. Responsibilities include triaging urgent needs, making home visits, performing skilled assessments, managing symptoms and medications, collaborating with interdisciplinary team members, and supporting families through education and emotional support.
