It is home to several Fortune 500 companies, including Northwestern Mutual, WEC Energy Group, Rockwell Automation, and Harley-Davidson.The Facility – Milwaukee VAMC The Clement J. Zablocki VA Medical Center is located on 245 acres on the western edge of Milwaukee and part of VA Integrated Services Network 12 (VISN 12), which includes facilities in Iron Mountain, Michigan; Tomah and Madison, Wisconsin, and North Chicago, Hines, and Chicago in Illinois. The Medical Center delivers primary, secondary, and tertiary medical care in 196 hospital beds, and provides more than 857,000 visits annually through an extensive outpatient program. The nursing home care unit of 113 beds offers geriatric programming and the 150 domiciliary beds are the focus of programs in Substance Abuse Rehabilitation, Psychiatric Rehabilitation and Post Traumatic Stress Disorder care.Essential Duties and Responsibilities The RN will serve as a Community Care Nurse Coordinator and will utilize their broad based clinical knowledge to manage basic, moderate, and complex care of the Veteran and the seamless transition to and from the community and to serve as a liaison to both internal and external Providers. The Community Care Nurse Coordinator functional role includes organizing and coordinating clinical components for the authorization of non-VA services in response to individual healthcare needs along the illness and care continuum and in multiple settings. Duties included but not limited to:Supports the Veterans referred for evaluation, coordinates between the Medical Center Providers and community Providers, and ensures appropriate follow-up consults are entered by the Provider.Coordinates patient appointments, conducts follow-up, reports test results to the appropriate Providers, and obtains records and test results from outside facilitiesCollaborates effectively with all levels of the health care team. This includes, but is not limited to clinics, inpatient departments, physicians, nurses, and other health care providers such as pharmacists, social workers, psychologists, physical therapists, dieticians, other outpatient departments, ancillary services, community Providers, etcEnsures instruction to the patient and the family is based on identified learning needs. Assesses the family's knowledge base, health status expectations, and the potential for the family member acting as the primary caregiver if necessaryAssists with the development of activities and methods to ensure information is articulated and disseminated to staffProvides tracking and trending of referrals related to authorizations, billing, and clinical outcomesDocuments patient's progress on clinical pathways/algorithmsPlans care based on patient needs and health care needs, at home and in the health care settingUtilizes nursing interpretations to alert physicians to significant changes or abnormalities and provides medical information about the patient's condition, medical history and specialized treatment plan or protocol to other professionals.Applies critical thinking and analytical skills to identify barriers for care deliveryRequirements:ASN or BSN from an ACEN or CCNE accredited Nursing program.Current and unrestricted license in a State, Territory, or Commonwealth of the United States or the District of ColumbiaEvidence of completion of required licensure, credentials, required training, current competencies and fingerprints/background investigations must be validatedShift: Minimum 8-hour shifts (typically 7:30am-4:00pm including a 1/2 hour non-paid lunch) Mon-Fri.KurzSolutions is committed to improving health outcomes by providing well-managed companies with the top healthcare talent in the market.
