Key Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
Provide therapeutic interventions to improve patients' functional independence in daily living activities
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to ensure coordinated patient care
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

Active Occupational Therapist (OT) license in the state of Hawaii or eligibility to obtain one
Graduate of an accredited Occupational Therapy program
Experience working in a skilled nursing facility or similar post-acute care environment
Strong clinical documentation and communication skills

Preferred Experience / Skills

Prior travel or contract therapy experience
Familiarity with MDS and skilled nursing facility regulatory requirements
Experience with a variety of patient populations including geriatric care
