PT (Physical Therapist)

Provides physical therapy services to patients/clients according to a written physician's plan of care, including assessing, evaluating, and updating therapeutic/rehabilitative/functional status.

Responsibilities:

• Assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participating in the development of the total plan of care.
• Evaluates home environment. Assesses for muscle strength, mobility, gait, ROM - potential for rehab. Directs physical therapy treatment.
• Instructing patients/clients and families/caregivers in the use and care of therapeutic appliances.
• Reporting the patient's/client's reaction to treatment or changes in condition to the physician.
• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy.
• Supervising homemaker/Home Health Aides when such personnel are participating in patient's/client's rehabilitation regime.
• Identifies patient/client and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Participates in and completes ongoing staff competency assessment.

Requirements:

• Minimum of 2 years experience as a PT in an acute/home care/rehab setting.
• Required certifications: BLS Certification.
• Good oral and written communication skills; has good organizational skills.
• The service area is Yolo County, which includes Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, Esparto.
• Scrubs are not required; if scrubs are not worn, the dress code is slacks (khakis are fine) and polo or button-up shirt.
• Candidates may not have been directly employed by any facility associated with CommonSpirit within the past year.
• Charting System: Homecare Homebase (HCHB).
• License to Submit: Yes.
