We are seeking a compassionate, experienced, and dynamic Hospice Clinical Director to lead and oversee the delivery of high-quality hospice care services. The Clinical Director is responsible for the overall management, coordination, and supervision of hospice clinical operations, ensuring compliance with federal, state, and accreditation standards while promoting exceptional patient and family experiences.
The ideal candidate will provide leadership and support to interdisciplinary team members, including nurses, social workers, chaplains, hospice aides, and other clinical staff. This role is responsible for maintaining clinical excellence, driving quality outcomes, supporting staff development, and ensuring patients receive individualized, compassionate end-of-life care. Qualifications:- Active Registered Nurse (RN) license in good standing.
- Bachelor's degree in Nursing required; BSN preferred.
- Minimum of 3-5 years of hospice leadership or management experience.
- Thorough knowledge of hospice regulations, Medicare guidelines, and accreditation standards.
- Strong leadership, communication, organizational, and problem-solving skills.
- Demonstrated ability to lead interdisciplinary teams and foster a culture of compassion and accountability.
- Direct and supervise all clinical hospice services and patient care activities.
- Ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, and agency policies.
- Provide leadership, coaching, and performance management for clinical staff.
- Oversee patient care planning and interdisciplinary team meetings.
- Monitor quality assurance and performance improvement initiatives.
- Collaborate with physicians, community partners, and leadership to ensure continuity of care.
- Manage clinical staffing, scheduling, productivity, and resource utilization.
- Investigate and resolve patient, family, and staff concerns in a timely and professional manner.
- Support census growth through clinical excellence and strong referral source relationships.
- Maintain accurate documentation and ensure adherence to electronic medical record requirements.
