Job Summary Under minimal supervision and at the direction of the Case Management Director, the Inpatient Social Worker/Patient Counselor supports patients in addressing their bio-psychosocial needs, facilitating access to medical care and community resources to optimize their health outcomes. This role involves crisis intervention, short-term counseling, and transition planning, ensuring patients move smoothly along the continuum of care. The Social Worker/Patient Counselor also coordinates discharge planning by collaborating with patients, families, and healthcare teams. Ideal Candidate Profile The ideal candidate is a compassionate and resourceful professional with strong problem-solving skills. They demonstrate expertise in crisis intervention, patient counseling, and care coordination while maintaining a patient-centered approach. Key Responsibilities Conduct bio-psychosocial assessments to determine patient needs. Provide crisis intervention and short-term counseling for patients and families. Assist patients in accessing medical care and community resources . Develop and execute discharge plans , collaborating with healthcare teams. Support patients transitioning across different levels of care. Ensure compliance with healthcare regulations and ethical standards. Maintain accurate documentation of patient interactions and treatment plans. Required Qualifications Master's Degree in Social Work or Counseling Psychology .
