Hospice Field RN Case Manager: Impactful Home-Based Care

MJHS Hospice and Palliative Care is seeking a Field Case Manager to visit patients in community and facility settings, perform physical and psychosocial assessments, and document visits electronically. You will develop and implement nursing care plans and make recommendations tailored to patient needs. The role requires NYS RN license and NYS Driver’s license, with a schedule that includes one weekend per month and backup On Call 2–3 times monthly. #J-18808-Ljbffr MJHS
