Job Summary

We are seeking a highly skilled and dedicated Registered Nurse with Certified Diabetes Educator (CDE) credentials to join our team at Tsehootsooi Medical Center in Fort Defiance, Arizona. This role involves specialized nursing work within our Diabetes Clinic, focusing on comprehensive diabetes care.

The successful candidate will be instrumental in providing consultation, assessment, and evaluation of diabetes-related services. This includes working with individuals, families, and communities to manage and improve health outcomes related to diabetes, potentially also supporting primary care clinic needs.

Key Responsibilities

• Provide expert consultation for individuals, families, and communities regarding diabetes-related services.
• Conduct thorough assessments of patient needs and current diabetes management strategies.
• Evaluate the effectiveness of diabetes care plans and educational interventions.
• Develop and implement individualized care plans to promote optimal patient health and self-management.
• Educate patients and their families on diabetes prevention, management, and lifestyle modifications.

Required Qualifications

• Minimum of three (3) years of recent, direct experience in a Diabetes Clinic or similar specialty setting.
• Mandatory clinic experience.

Shift Details

• Hours: 8 a.m. to 5 p.m.
• Days: Monday through Friday.

• Assignment Dates: August 25, 2026, for a 13-week contract.

Licensing Requirements

• Active Registered Nurse (RN) License.
• American Heart Association (AHA) Basic Life Support (BLS) certification.
• American Heart Association (AHA) Advanced Cardiovascular Life Support (ACLS) certification.
• Current Certified Diabetes Educator (CDE) certification, obtained within the last three (3) years of the start date.
