About the Position

Job Title: Registered Nurse I Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and OASIS Description: Provides coordinates and directs the provision of home nursing care according to physician's orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Home visits are required. Periodic office meetings are part of the job. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Contractor will be assigned between 5-10 patients per day. This equates to roughly 6-8 points. Different visit types and activities hold various point values. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-timers are not accepted; recent specialty experience is required. Weekend requirements vary by location and need to be discussed during the interview. On-call requirements vary by location and should also be discussed during the interview. Must be willing to travel up to 75 miles from the assigned care center. Mileage will be reimbursed at the IRS standard rate. Local candidates are accepted under the same conditions. No pending licenses will be accepted; a valid license is needed. Must be proficient in HomeCare HomeBase and OASIS. Documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts follow-up patient care will result in a cancellation. Understanding that there are no guaranteed hours with this contract is essential. Weekly productivity points of 30 are expected, equating to 40 hours per week. Each contractor must accept all patients on their schedule, regardless of distance. Declining patients may lead to a cancellation.
