Submission Requirements

• 2 years current FIRST ASSIST OPERATING ROOM experience (Required)
• Bachelor of Science degree (Required)
• BLS AHSA (Required)
• CNOR REQUIRED (Required)
• Meditech Expanse experience (highly preferred)
• Perm address must be included in profile
• Traveler must have their own vehicle for this need
• Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION.

• Work History
• Skills Checklist, FIRST ASSIST OPERATING ROOM, current to 1 year (REQUIRED) (DOCUMENT)
• 1 Supervisory Signed Reference (from within the past two years of employment) (REQUIRED)
• Massachusetts RN license - Must be uploaded with profile (REQUIRED) (DOCUMENT)
• Active American Heart Association BLS - (REQUIRED) (DOCUMENT)
• Active Certified Perioperative Nurse CNOR - (REQUIRED) (DOCUMENT)
