About the Role As a Licensed Practical Nurse (LPN) in our Home Care & Hospice team, you will be more than a clinician; you will be a trusted presence in some of life’s most vulnerable moments. Your care will help patients maintain dignity, independence, and comfort in their own homes. You’ll be a vital link between patients, families, and the broader care team, ensuring that every individual receives the attention, compassion, and expertise they deserve. What You'll Do Provide skilled nursing care in patients’ homes, including medication administration, wound care, catheter care, and symptom management. Monitor and document patient conditions, promptly reporting changes to the RN Case Manager or physician. Offer emotional support and education to patients and families, helping them understand care plans and manage symptoms. Collaborate closely with interdisciplinary team members—RNs, social workers, chaplains, and therapists to ensure holistic care. Assistwithend‑of‑life comfortmeasures, honoring patient wishes and supporting families through the hospice journey. Maintainaccurate, timely clinical documentation in compliance with regulatory and agency standards. Qualifications Current, active LPN license in the states of NH and Maine (employer will assist in applying for Maine License). At least 1 year of nursing experience; home health or hospice experience preferred. Strong clinical assessment skills and the ability to work independently in varied home environments. Compassion, patience, and the ability to build trust quickly with patients and families. Reliable transportation and a valid driver’s license. Comfort with electronic medical records and mobile documentation tools.
