Hospice Case Manager

You will collaborate closely with physicians, social workers, chaplains, and other team members, ensuring seamless care coordination and ongoing support for families. Responsibilities include symptom and pain management, patient and caregiver education, accurate EMR documentation, and connecting families with community resources.
