Type of Facility: Skilled Nursing Unit as part of an acute hospital

Room Type: Semi-private rooms

Charting System: Cerner

Ratios: 5 RNs to 1 LPN/CNA

Common Diagnoses/Types of Patients: Pulmonary diagnoses requiring oxygen Alzheimer's Dementia

Job Duties: RNs can pass medications Wound care Tracheostomy care IV care

Experience: No IVs or titratable drips

Monitoring: No tele monitors; RNs do not read strips

Attendance at team conferences for acute inpatient rehab is required

House Supervisor is available Physician availability after hours is confirmed

Interdisciplinary Team: Physical Therapy Occupational Therapy Speech Language Pathology Pulmonary or Cardiac Rehab Case Managers Social Workers Dietitians

Additional Notes: Respiratory Therapy supports stable vents and trach care Oxygen Therapy/Equipment (e.g., CPAP, BiPAP, High Flow) is available on-site Pharmacy on-site with medication dispensing system

Shifts & Scheduling: Self-scheduling or block scheduling is not allowed Weekend and holiday requirements are necessary No call or standby requirement Floating requirements to Acute Rehab and MedSurg/Tele

Scrub Color/Dress Code: RN scrubs: Navy Blue LPN scrubs: Navy Blue CNA scrubs: Forest Green

Unit/Department Specific Orientation is provided PAPR or N95 masks are utilized Cancellation policy allows for certain shifts to be canceled during assignments Trauma Level: Varies based on care setting Care Setting: Post Acute
