We are seeking a Registered Nurse (RN) for a Home Health role focused on patient-centered care in a community setting. Responsibilities: Evaluate patient status, develop and implement care plans, provide direct treatment and education, and coordinate with multidisciplinary teams to promote health, prevent illness, and manage medical conditions at home. Key Duties: Monitor patient conditions, communicate changes, make care recommendations, and facilitate referrals to community resources. Work Environment: Flexible scheduling with a smaller caseload, support from triage and education teams, limited on-call and after-hours responsibilities, and opportunities for professional growth. Qualifications And Skills Active Maine RN license or ability to obtain one. At least 1 year of acute or med/surg nursing experience; ED, ICU, CCU, or home care preferred. Valid driver’s license with an acceptable driving record. Strong technology skills and experience with electronic medical records (EMR) preferred.
