Job Summary:

Responsibilities:

• Provide customized care and support to clients in their homes, focusing on their physical, emotional, and social well-being

• Assist with daily living activities, including personal care, medication management, and light housekeeping

• Develop and maintain strong relationships with clients and their families, promoting open communication and trust

• Collaborate with other healthcare professionals to ensure seamless care transitions and continuity of services

• Maintain accurate and timely records of client care and services provided

Requirements:

• 1+ years of experience in a healthcare setting, preferably in home care

• CNA preferred but not required

• Ability to lift, push, or pull up to 50 pounds and assist with transfers and ambulation

• Reliable transportation and a valid driver's license

• High school diploma or equivalent required; certification in CPR and First Aid a plus
