Advanced Practice Provider (PA or NP)

Geriatric House Calls – Assisted Living Focus

We are seeking a compassionate and motivated Physician Assistant (PA) or Nurse Practitioner (NP) to join our geriatric house calls program , providing longitudinal primary and supportive care to residents of assisted living and memory care communities.

This role is ideal for an APP who values time with patients, continuity of care, and meaningful long-term relationships —not volume-driven medicine.

You will manage a defined patient panel, work closely with facility staff and families, and practice with the support of a highly organized clinical and administrative team.

• Low daily visit volume : ~10 patients per day on average

• Relationship-based care with long-term patient panels

• Full-time dedicated administrative medical assistant

• 24/7 call center support (triage, message handling, escalation)

• Providers are on call , but do not need to immediately answer calls

• No rushed visits, no assembly-line medicine

• Strong interdisciplinary collaboration with nursing, families, and facilities

• Provide comprehensive primary and geriatric care to assisted living residents

• Manage and take ownership of an assigned patient panel

• Perform in-person visits in assisted living and memory care settings

• Develop and maintain long-term therapeutic relationships with patients and families

• Coordinate care with facility staff, specialists, and support services

• Participate in 24/7 call for your patient panel with call-center triage support. Call is from home and no in person or telehealth visits after hours are expected.

• Maintain accurate, timely clinical documentation

• Shared 24/7 call coverage for your patient panel

• Call center triages first and escalates only when clinically appropriate

• Focus is on continuity and accountability , not constant interruptions

• Designed to be sustainable and provider-friendly

• Licensed Physician Assistant or Nurse Practitioner (Florida)

• Experience in geriatrics, primary care, internal medicine, or long-term care preferred

• Comfortable practicing independently with physician collaboration

• Physician available 24/7 for escalation and ongoing education

• Strong clinical judgment and communication skills

• Passion for caring for older adults and building long-term patient relationships

• Reliable transportation for local travel between facilities
