Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Conduct comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis, past history, precautions, and contraindications
• Establish measurable goals and develop treatment plans
• Regularly reassess treatment plans and patient progress
• Communicate patient needs and progress to the treatment team and others as applicable
• Consult with medical providers about patient evaluation, treatment, and progress
• Recommend and assist in securing adaptive equipment for patient use
• Maintain and submit evaluation, treatment, and billing documentation as required
• Adhere to professional standards and laws
• Supervise Physical Therapist Assistants and Aides
• Participate in professional development and internal staff programs

Requirements of the Physical Therapist

• West Virginia Physical Therapy License (WV PT)
• Home Health experience with OASIS preferred
• Experience with HCHB is preferred
