Registered Nurse / LPN - Adult Foster Care Program

Position Summary The Adult Foster Care (AFC) Nurse provides clinical oversight, home-based assessment, and coordinated support services for individuals with medical needs and/or developmental disabilities within the AFC program. This role utilizes a collaborative, team-based approach to ensure high-quality, person-centered care and regulatory compliance. LPNs encouraged to apply as well. Schedule: 24 hours/week Compensation: $36 - $38/hour Key Responsibilities Clinical Oversight & Service Delivery Participate in the evaluation of potential AFC members and prospective caregivers. Conduct clinical assessments and develop individualized, multidisciplinary care plans. Provide clinical supervision and ongoing support for assigned AFC placements, including monthly home visits and case management. Monitor participant health status on a continuous basis and respond to changes promptly. Provide education to members and caregivers regarding medical concerns, prevention strategies, and wellness practices. Coordinate community medical services and personal care supports for members as needed. Collaborate with primary care providers, community service providers, medical professionals, and internal team members to ensure integrated care. Documentation & Compliance Complete and/or review all required AFC program documentation in accordance with agency standards. Maintain compliance with Kennedy-Donovan Center mission, philosophy, policies, procedures, and quality standards. Ensure adherence to all applicable state and federal funding agency certification requirements and regulations. Maintain certifications and competencies required by the agency and funding sources. Collaboration & Communication Serve as a resource for caregivers, case managers, and interdisciplinary team members regarding clinical needs. Communicate effectively with primary care providers, caregivers, families, and program staff to share updates on participant health and support needs. Professional Development & Additional Duties Participate in professional development activities to enhance clinical skills and maintain licensure. Perform other job-related responsibilities as assigned. Required Qualifications Current Massachusetts Registered Nurse (RN) licensure. Minimum of 3 years of experience in human services, long-term supports, disability services, or a related program area. Understanding of home- and community-based service models, AFC, Shared Living, or related MassHealth programs. Excellent communication, organizational, and interpersonal skills. Proficiency with Electronic Health Records (EHR), and Microsoft Office Suite. Valid driver’s license and reliable transportation for travel. Ability to complete all required trainings such as CPR and First Aid. Preferred Qualifications Experience working in community-based programs. Experience with Medicaid, MassHealth, and state funding systems. Bilingual or multilingual skills. #J-18808-Ljbffr
