The National Health Service Core (NHSC) Loan Repayment Program is available for licensed primary care medical, dental, and mental and behavioral health providers employed by accredited sites. About the Role The Case Manager works as a member of an interdisciplinary team to build working relationships, solve problems, and support individuals seeking and receiving services. Responsibilities primarily include guiding, assisting, and supporting individuals with psychiatric, substance abuse, medical, social, and legal issues to improve daily living skills and advance recovery. The Case Manager collaborates closely with KBBH therapists, psychiatric providers, community partners, and client medical providers. The position participates in program goals and objectives to promote a strengths‑based, wellness‑informed, client‑centered approach to achieving consumer‑driven and medical goals. Key Responsibilities Serve as an active member of the treatment team through effective communication with clinicians, social service agencies, and monitoring bodies. Communicate relevant clinical and service‑related information to support coordinated care. Assist individuals with applying for and maintaining insurance benefits. Coordinate or facilitate access to medical care. Assist with applying for and maintaining Social Security benefits. Support individuals in finding and maintaining stable housing. Coordinate transportation for appointments and daily needs. Assist with shopping for food, household, and personal items. Arrange for a representative payee when an individual cannot manage funds. Coordinate employment and educational opportunities. Arrange foster care services when required. Arrange personal care attendant services to support individuals at home or in the community. Manage medication, including observing medication box set‑up, assisting with pharmacy communications, and providing transportation to pharmacies. Provide training in personal and environmental hygiene, nutrition, cooking, shopping, and budgeting. Support development of communication skills, interpersonal effectiveness, coping strategies, and symptom management. Assist with community navigation, socialization, and employment or educational skill development. Promote recovery principles, dignity, hope, and overall well‑being. Foster community integration and encouragement of natural supports. Provide education and support to families and other natural supports to promote wellness and recovery. Advocate for clients with agencies, service providers, and stakeholders to support goal attainment and protect client rights. Use de‑escalation, redirection, empathetic listening, effective communication, and sound judgment when responding to individuals experiencing psychosis, mania, anger, or depression. Apply safety measures when responding to high‑risk situations. Provide or arrange transportation for clients to and from appointments as needed. Complete all internal and external documentation accurately, professionally, and within required timelines. Ensure that all services provided are listed on the client’s current treatment plan prior to service delivery. Monitor treatment plan expiration dates and coordinate with the therapist of record to update or renew plans as needed. Maintain accurate time and attendance records and manage time and scheduling amid competing priorities and unplanned tasks. Comply with HIPAA, Oregon Administrative Rules, and confidentiality laws; complete documentation within 48 hours or month‑end timelines. Engage in growth opportunities, meetings, and trainings. Success in This Role Looks Like Clients experience empathy, clarity, and consistent follow‑through. Referrals and documentation are completed accurately and on time. Effective communication with multiple providers and systems. Barriers to care are identified and reduced. Presence remains calm and steady during crisis situations. Education & Experience Must meet OARS and MHACBO.org requirements for QMHA certification. Candidates should have one of the following: Bachelor’s degree in a behavioral science field. A combination of at least three years of relevant work, education, training, or experience. Licenses & Certifications Valid Oregon driver’s license. Insurable driving record. Must meet MHACBO QMHA certification requirements and maintain certification for the duration of employment. Compensation: $23.55 - $29.45 hourly wage, with salary determined by level, requirements, education, experience, internal equity, and market data. Typically, new team members join at the minimum to mid salary range.
