Key Highlights Weekly pay: $1,901–$1,988 per week Assignment length: 13 weeks Shift: 5x8 Days Start date: 08/10/2026 Setting: Skilled Nursing Facility (SNF) / Long-Term Care Travel - Temporary Assignment through Healthforce Healthcare LLC Responsibilities Conduct initial and ongoing assessments of residents’ mobility, balance, strength, gait, and functional limitations. Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability, aligned with physician orders and facility policies. Deliver direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and the use of physical agents or modalities. Monitor resident progress and adjust care plans to meet therapy goals; participate in interdisciplinary care team meetings and contribute to discharge planning as appropriate. Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence. Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements. Accurately document treatment sessions, progress notes, and billing information in a timely manner; ensure compliance with state, federal, and facility documentation standards. Support quality assurance and performance improvement initiatives as part of the rehab team. Requirements Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the state of practice (Missouri). Experience in SNF or geriatric rehabilitation is preferred. Strong clinical, communication, and organizational skills. Ability to work independently and as part of an interdisciplinary team. Comfortable working in a skilled nursing/long-term care setting.
