Nurse Practitioner (NP) – Primary Care

Location: Oswego, New York
Job Type: Full-Time
Salary: $130,000 + Base (Open to discussions) 
Sign-On Bonus: $10,000–$20,000
Practice Setting: Outpatient Primary Care
Patient Population: Veterans

Position Overview

We are seeking a dedicated and compassionate Nurse Practitioner (NP) to provide comprehensive primary care services to veterans in Oswego, New York.

The Nurse Practitioner will evaluate, diagnose, and manage a wide range of medical conditions, develop individualized treatment plans, prescribe medications, and promote preventive healthcare. The ideal candidate will have primary care or ambulatory care experience and a strong commitment to delivering high-quality, patient-centered care.

Key Responsibilities

• Conduct comprehensive patient assessments, physical examinations, and medical evaluations.

• Diagnose and manage acute and chronic medical conditions within the scope of practice.

• Develop, implement, and monitor individualized treatment and care plans.

• Order and interpret diagnostic tests, including laboratory studies, X-rays, and electrocardiograms.

• Prescribe medications and recommend appropriate lifestyle, dietary, and preventive health measures.

• Educate patients about their diagnoses, medications, treatment plans, and preventive healthcare.

• Assess mental health concerns and coordinate additional care when necessary.

• Review outside medical records and coordinate care with other healthcare providers.

• Maintain accurate and timely clinical documentation, completing medical records within 24 hours of each patient encounter.

• Collaborate with physicians, registered nurses, medical assistants, and other members of the healthcare team.

• Ensure appropriate patient follow-up, referrals, and continuity of care.

• Respond to patient requests and clinical alerts in accordance with established protocols.

• Maintain compliance with applicable clinical guidelines, regulatory requirements, and quality standards.

• Participate in patient outreach, enrollment, and retention initiatives.

• Support clinical quality improvement and operational initiatives.

Qualifications

• Graduate of an accredited Nurse Practitioner program.

• Current national certification as a Nurse Practitioner through an applicable certifying organization.

• Current, unrestricted Registered Nurse (RN) and Nurse Practitioner licensure, as required by New York State and applicable employment requirements.

• Minimum five years of combined approved academic training and healthcare experience, or as otherwise specified by applicable contractual requirements.

• Primary care or ambulatory care experience required; government healthcare experience preferred.

• Current Basic Life Support (BLS) certification.

• Advanced Cardiovascular Life Support (ACLS) certification, if required by applicable clinical requirements.

• Current, unrestricted Drug Enforcement Administration (DEA) registration, as applicable.

• Eligibility for applicable Veterans Health Administration credentialing requirements.

• Strong written and verbal English communication skills.

• Proficiency with computers and electronic medical records
