Assignment Details Contract length: 13 weeks Shift length: 8 hour shifts Hours per week: 40 Start date: ASAP Job Responsibilities Evaluate functional status and develop a treatment plan aligned with clinician goals and patient needs. Communicate functional changes and barriers to the care team to support care planning. Educate patients and caregivers on exercises, safety strategies, and home programs as appropriate.
