That includes yours.You will report to the Associate Medical Director at our South Daytona Clinic.Why You'll Love Working HereTeam-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness.More Time with Patients: See fewer patients per day and spend more time delivering meaningful care.Supportive Culture: We foster a welcoming, inclusive environment where teamwork and growth are prioritized.Your Role & ResponsibilitiesYou will provide comprehensive care to seniors following clinical standards.You will collaborate daily with your care team to ensure coordinated, high-quality care.You will manage referrals, hospital/SNF coordination, DME, and home health services.You will document care with support from quality-based coders.You will participate in on-call rotation and support growth plans at your center.You will work on-site.What We're Looking ForRequired:Master's degree in nursing with board certification, or graduation from an accredited PA program with board certification.Active, unrestricted NP or PA license in the state of Florida.At least one year of experience in value-based care, primary care, or Medicare Advantage settings.Active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.Familiarity with HEDIS quality indicators and Medicare guidelines.Scheduled Weekly Hours40Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan.
