Registered Nurse, RN- Case Manager

Job Overview The RN Case Manager manages a designated caseload to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Schedule & Location Hours: 40 hours - weekend commitments and holidays required. Shift: Day shift. Location: Lowell General, Main Campus. Minimum Qualifications Massachusetts RN Licensure 3 Years of RN Experience Current Basic Life Support (BLS) Certification Preferred Qualifications Bachelor of Science in Nursing (BSN) Duties and Responsibilities Interview and collect specific information and data from patients and families; contribute to the identification of physical, emotional, educational, and cultural needs. Maintain documentation of assessment findings, discharge arrangements, and actions taken according to departmental guidelines. Complete a discharge assessment with appropriate documentation according to established policies and procedures. Identify barriers to efficient, cost-effective provision of care and expected individualized outcomes for the patient. Collaborate with physician, healthcare team, and patient/family in decision making and planning of care during hospitalization and in anticipation of discharge. Implement the plan of care to achieve the identified expected outcomes. Utilize community resources and systems to implement the plan, facilitating referrals when appropriate. Provide education to patients and families and document according to policy. Coordinate activities related to financial/insurance issues, including providing clinical updates to insurance companies and coordinating insurance appeals related to discharges while adhering to Medicare and Medicaid policies and procedures. Assist in achieving unit quality scores that meet or exceed hospital established targets for nurse-sensitive quality indicators as appropriate for the unit.
