Contract | 13 Weeks | Days | Monday–Friday | 24–40 Hours Per Week

Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay
• Delegate and direct all discipline-specific therapy services for assigned patients
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adhere to the applicable State Practice Act and all regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements

Required Qualifications

• Valid Georgia Physical Therapist license in good standing
• Ability to evaluate patients and develop clinically appropriate, goal-directed treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
• Commitment to patient and employee safety standards and safe patient handling practices

Preferred Experience / Skills

• Prior outpatient physical therapy experience preferred; new graduates will be considered
• Experience with performance improvement initiatives and professional development activities
