Description: The Case Manager role involves providing care coordination and discharge planning for patients. Registered Nurses will work with a patient ratio of 1:20. The position requires participation in family meetings and contributing to the treatment plan development. Recent experience in acute care settings is required, demonstrating competency in handling patients with complex needs. The ideal candidate must be proficient in computerized documentation and have experience with charting systems. Ability to work collaboratively with an interdisciplinary team, including social services, is essential. Must be adaptable to meet the department's needs, including weekend rotations as required. Candidates should possess strong organizational and communication skills to effectively navigate patient care processes. Willingness to float to nearby facilities may be necessary.
