Essential Job Functions

• Provide admissions coverage at Integritus Healthcare facilities in Bourne, Yarmouth, and New Bedford, with assignments based on operational needs.

• Coordinate the resident admission process from initial referral through admission.

• Collect and review clinical and financial information, confirm payer sources, and obtain required approvals.

• Schedule and conduct tours and communicate with prospective residents, families, and referral sources.

• Complete admission paperwork and ensure OBRA, PASRR, and other admission requirements are satisfied.

• Coordinate new admissions with nursing, rehabilitation, social services, dietary, the Business Office, and other departments.

• Maintain accurate referral and admission records and provide timely updates to facility leadership.

• Support census goals, customer service, referral relationships, and facility marketing efforts.

• Identify and communicate admission barriers or process concerns and work with facility leadership to resolve them.

• Follow all confidentiality, resident rights, safety, infection control, and company requirements.

Qualifications

• High school diploma or equivalent required; bachelor’s degree preferred.

• At least three years of long-term care or related healthcare experience preferred.

• Admissions, referral management, or skilled nursing experience strongly preferred.

• Working knowledge of Medicare, Medicaid, OBRA, PASRR, and long-term care admission requirements.

• Strong organizational, communication, customer service, and problem-solving skills.

• Ability to work independently, manage competing priorities, and collaborate with interdisciplinary teams.

• Proficiency with basic computer applications and referral-tracking systems.

• Valid driver’s license, reliable transportation, and the ability to travel regularly between Bourne, Yarmouth, and New Bedford.

• Flexibility to change facility assignments based on business needs.
