Physical Therapist Assitant- HomeCare MD

LifeBridge Health is seeking a dedicated Physical Therapist Assistant – HomeCare MD to join our Healthcare & Medical Services team. As part of a leading regional healthcare organization in Maryland, you will provide high-quality, compassionate physical therapy services to patients in their homes, supporting a comprehensive continuum of care that extends from our hospitals and specialty centers into the community. In this role, you will work under the direction and supervision of a licensed Physical Therapist to implement individualized treatment plans for adult and geriatric patients recovering from surgery, illness, injury, or managing chronic conditions. You will travel to patients’ residences across assigned service areas in Maryland to deliver evidence-based therapeutic interventions that promote mobility, strength, balance, safety, and functional independence in the home environment. Your day-to-day work will include carrying out prescribed exercises and manual techniques, instructing patients and caregivers in proper use of assistive devices and home exercise programs, monitoring patients’ responses to treatment, and promptly reporting clinical changes to the supervising Physical Therapist and interdisciplinary team. You will perform detailed and timely documentation of all visits, interventions, and patient progress in the electronic health record, adhering to LifeBridge Health policies, professional standards, and payer and regulatory requirements. Collaboration is central to this position: you will communicate frequently with physical therapists, nurses, physicians, social workers, and other team members to coordinate care, reinforce safety plans, and support smooth transitions across care settings. You will also educate patients and families about disease processes, fall prevention, energy conservation, and strategies to maintain function and quality of life at home. The ideal candidate is patient-centered, organized, and comfortable working independently in the field while remaining closely connected to a supportive interdisciplinary team. You should demonstrate excellent clinical reasoning, strong interpersonal and communication skills, and a commitment to culturally sensitive care in diverse communities. You will be expected to manage a daily caseload, effectively plan travel routes, and adapt treatment sessions to variable home environments and equipment constraints. You will also participate in case conferences, performance improvement initiatives, and continuing education activities that reflect LifeBridge Health’s culture of collaboration, innovation, and continuous improvement in healthcare delivery.
