Senior Nurse Consultant (RN)

PruittHealth is seeking an experienced Senior Nurse Consultant (RN) to support our skilled nursing and long-term care centers throughout South Georgia and North Florida. This highly visible role is ideal for a clinical leader who is passionate about rapid response intervention, facility turnaround efforts, regulatory recovery, survey readiness, quality improvement, and leadership coaching.

Unlike a traditional compliance-focused consulting role, this position serves as a critical resource for facilities experiencing survey challenges, quality concerns, operational instability, leadership transitions, or regulatory risk. The successful candidate will quickly assess clinical and operational performance, identify root causes, implement corrective actions, and partner with facility leadership teams to drive sustainable improvement.

Key Responsibilities

• Rapid Response & Facility Recovery
  • Deploy to facilities requiring immediate clinical, operational, or regulatory intervention.
  • Conduct comprehensive facility assessments to identify compliance deficiencies, operational risks, and quality concerns.
  • Lead stabilization efforts for facilities experiencing adverse survey outcomes, complaint investigations, Immediate Jeopardy risks, or significant regulatory challenges.
  • Develop and execute recovery plans with measurable outcomes and clear accountability.
  • Partner with Administrators and Directors of Health Services to prioritize corrective actions and improve facility performance.
• Regulatory Compliance & Survey Readiness
  • Conduct comprehensive mock surveys in accordance with CMS and state regulatory standards.
  • Evaluate clinical, environmental, and life safety compliance practices.
  • Review clinical documentation, including MDS assessments, care plans, medical records, and medication administration records.
  • Identify potential deficiencies and implement corrective action strategies.
  • Assist facilities with Plans of Correction (POCs) and ongoing Quality Assurance Performance Improvement (QAPI) initiatives.
  • Monitor corrective actions and validate sustained compliance and readiness.
• Quality Improvement & Clinical Excellence
  • Analyze quality measures, CASPER reports, infection prevention data, staffing metrics, incident trends, and resident outcomes.
  • Lead Root Cause Analysis (RCA) activities and support QAPI initiatives.
  • Identify opportunities to improve Five-Star performance, resident outcomes, and regulatory compliance.
  • Support facilities in establishing systems that promote long-term success and continuous survey readiness.
• Leadership Coaching & Development
  • Coach and mentor Administrators, Directors of Health Services, Assistant Directors of Nursing, MDS Coordinators, Infection Preventionists, and interdisciplinary leadership teams.
  • Support newly appointed or developing clinical leaders through education, accountability, and hands-on guidance.
  • Strengthen leadership effectiveness during periods of organizational change or operational recovery.
• Professional Development
  • Participate in clinical leadership meetings, regulatory readiness calls, and educational initiatives.
  • Maintain current knowledge of CMS regulations, F-Tags, survey processes, infection prevention standards, and long-term care best practices.

Qualifications

Required

• Active, unrestricted Registered Nurse (RN) license with multistate eligibility.
• CPR Certification.
• Minimum 5 years of clinical leadership experience within skilled nursing or long-term care.
• Minimum 3 years of experience in a leadership role such as:
  • Regional Nurse Consultant
  • Senior Nurse Consultant
  • Corporate Clinical Consultant
  • Director of Health Services (DHS)
  • Director of Nursing (DON)
  • Quality Improvement or Regulatory Compliance Leader
• Demonstrated experience leading facility turnaround efforts, rapid response initiatives, regulatory recovery projects, or survey correction activities.
• Extensive experience with CMS regulations, F-Tags, survey readiness, QAPI programs, compliance audits, and Plans of Correction.
• Experience supporting multiple facilities and managing competing priorities.

Preferred

• Experience managing facilities with high-risk survey outcomes, complaint investigations, or Immediate Jeopardy situations.
• Infection Preventionist certification.
• Wound Care certification.
• Strong analytical, auditing, problem-solving, and data interpretation skills.
• Proven success improving quality outcomes and regulatory performance across multiple facilities.

MINIMUM EDUCATION REQUIRED:

Graduate of an accredited educational institution with a degree in Nursing

Continuing education in regulatory compliance, infection control, or Quality Assurance Performance Improvement is a plus.

MINIMUM EXPERIENCE REQUIRED:

Minimum of 3-5 years of clinical experience in a long-term care or skilled nursing facility

At least 2 years of leadership experience (Director of Health Services, Assistant Director of Nursing, Quality Nurse, or Clinical Consultant).

Prior experience conducting mock surveys or participation in state/federal survey processes (preferred)

Familiarity with Plans of Correction, Quality Assurance Performance Improvement programs and compliance audits

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

Must possess an active Registered Professional Nursing License (multi-state)

Infection Preventionist certification preferred, but not required

CPR Certification

ADDITIONAL QUALIFICATIONS: (Preferred qualifications)

Ability to analyze data, identify trends and implement improvement strategies

Strong understanding of care coordination, patient experience and quality improvement principles

Ability to lead and manage teams across multiple locations
