MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Personal Care Assistant (PCA) In-Home Care

Location: Leominster & Surrounding Areas
Schedule: Flexible Hours | Based on Availability
Wage: $18.50/hour + Mileage Reimbursement

Applicants must have:

• Valid full Driver’s License held for at least 12 months
• Reliable vehicle for traveling to individuals’ homes
• Valid and verifiable Personal Care Homemaker (PCHM) Certificate

Join our team as a Personal Care Assistant (PCA) and provide one-on-one support to individuals in their own homes. You’ll help clients maintain their independence, safety, comfort, and daily routines through personal care, household assistance, meal preparation, errands, and companionship.

Key Responsibilities

• Assist with personal care , including bathing, dressing, grooming, and toileting
• Support mobility and other activities of daily living
• Cleaning, laundry, organizing, and light household tasks
• Meal preparation and meal planning
• Grocery shopping, errands, and essential household needs
• Provide companionship and support with daily routines
• Document services using a mobile device
• Communicate concerns or changes to appropriate team members

Qualifications

• High school diploma or equivalent
• Must be at least 18 years old
• Valid Personal Care Homemaker (PCHM) Certificate required
• Valid Driver’s License held for at least 12 months and reliable vehicle required
• Must pass required background checks
• Must complete a physical exam and TB skin test
• Dependable, compassionate, organized, and able to work independently
• Previous personal care or caregiving experience is a plus
