Job Title: Pediatrician (MD) Location: Brooklyn, New York
Reports To: Medical Director Position Description Under the supervision of the Medical Director, the Pediatrician will provide comprehensive, community-based primary care to infants, children, and adolescents in an patient-centered setting. - Provide full-scope pediatric primary care, including preventive visits, acute care, and chronic disease management.
- Manage a consistent patient panel with an emphasis on continuity of care and long-term health outcomes.
- Conduct thorough assessments and make independent clinical decisions across a wide range of pediatric conditions.
- Provide clear, practical health education to patients and caregivers, often tailoring guidance to varying levels of health literacy.
- Collaborate closely with nursing, care coordination, and behavioral health teams to support whole person care.
- Take ownership of follow-up care, including labs, referrals, and hospital transitions.
- Participating in immunization initiatives and community health efforts
- Maintain accurate, timely documentation in the EHR.
- Contribute to Quality Improvement initiatives focused on access, outcomes, and patient engagement.
- Supervise and support advanced practice providers and clinical staff as needed.
- Participate in team meetings and contribute to a collaborative, mission-driven culture.
- Other duties as assigned.
- MD from an accredited institution.
- Board Certified or Board Eligible in Pediatrics
- Active New York State medical license and DEA registration
- BLS or PALS certification
- Experience in community health, outpatient primary care, or similar settings strongly preferred.
- Demonstrated interest in working with underserved or high need populations.
- Comfortable practicing broad-spectrum pediatrics without reliance on extensive subspecialty support
- Strong commitment to community health and reducing healthcare disparities.
- Comfortable in a high-touch, high-accountability care model with direct patient responsibility
- Ability to work effectively in a resource-limited environment while maintaining high-quality care.
- Adaptable and solutions-oriented with a hands-on approach
- Excellent communication skills, especially with diverse and multicultural populations
- Collaborative mindset with willingness to collaborate across disciplines.
- Efficient and organized in a moderate-to-high volume outpatient setting.
- Community-based patient-centered setting serving a diverse patient population.
- Emphasis on access, continuity, and relationship-based care
- Physicians are expected to take an active role in patient follow-up and care coordination.
