RN Care Manager — Advanced Primary Care Leader

University Hospitals is seeking an RN Care Manager for Advanced Primary Care in Chardon, Ohio. This role involves applying nursing judgment, managing patient care across the continuum, and assisting patients with chronic disease management. The ideal candidate will have 3+ years of clinical experience, an accredited nursing degree, and strong communication skills. A Bachelor’s Degree and experience in outpatient settings are preferred.
