Hospice RN Case Manager: Compassionate Home-Based Care Leader

LilyCare Hospice in Florham Park, NJ, is seeking a full-time Hospice Registered Nurse Case Manager to lead care for patients in homes and facilities. You’ll perform assessments, develop care plans, and coordinate with physicians and the interdisciplinary team in a ground-up hospice program.
Requirements include an active NJ RN license (or compact state), at least two years of RN experience (hospice/home health/case management preferred), current CPR certification, and a valid driver's license.
