Requirements

• Current state license as a Registered Nurse - Required
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required
• CHPN certification - Preferred
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
• Bachelors of Science in Nursing - Preferred

Responsibilities

• Assess home health and/or hospice patients to identify their needs.
• Implement and document the Plan of Care (POC) to ensure quality care.
• Educate patients and families regarding care, medications, and self-care practices.
• Supervise and provide clinical direction to home health aides and support staff.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Provide effective communication with patients, families, and healthcare team members.
• Perform on-call responsibilities as assigned.
