Location:  White Plains, NY

Hours Per Week: 40-hour schedule (Sunday: 10:00 AM – 6:00 PM, Monday–Thursday: 10:00 AM – 6:00 PM)

FLSA Status:  Full-time ,  Non-exempt 

Salary Range : $22-24 per hour

Summary Description:  The Sr. Treatment Apartment Counselor is a pivotal role within the mental health services team, focused on providing counseling and support to individuals with mental health challenges. This position plays a key part in promoting recovery, independence, and overall well-being for our residents.

Responsibilities: 
Treatment Apartment: 

• Provide support, restorative services and skills training to culturally diverse adults recovering from mental illness in community-based treatment apartments.
• Assist residents in community integration and recovery; support residents in recovery processes.
• Provide transportation for residents with an agency vehicle when necessary.
• Conduct comprehensive assessments to identify residents' strengths, needs, and rehabilitation goals.
• Develop individualized service plans in collaboration with residents, focusing on skill-building and goal attainment.
• Provide individual and group counseling to support residents in achieving their independent living goals.
• Assist residents in developing and enhancing daily living skills, social skills, and coping mechanisms.
• Collaborate with a multidisciplinary team to ensure coordinated and holistic care.
• Monitor and document residents' progress, adjusting treatment plans as needed within established time frames,
• Advocate for residents' rights and access to community resources.
• Participate in treatment planning and case conferences to review and update resident goals.
• Stay informed about relevant developments in residential and mental health services.
• Other related duties as assigned.

Planned Respite: 

• Field phone calls. Decide if recipient is suitable for GMH's Planned Respite Program. If so, facilitate the admission process. If not, refer them to other agencies that are 24/7

Community residences:

• Monitor medication as needed
• Log in their information into the system (clear point). This includes attendance, even if at another facility.
• Submit bills for payment.
• Write a small summary of each resident's stay.
• Other related duties as assigned.

• Ability to conduct comprehensive assessments and develop treatment plans.
• Strong collaboration and communication skills within a multidisciplinary team.
• Demonstrated empathy and compassion in supporting residents.
• Advocacy for residents' rights and access to community resources.

Key Performance Indicators (KPIs):

• ISP Goal Progress Rate--Percentage of resident goals met as defined in Individual Service Plans within specified review periods.
• Resident Housing Retention--Number/percentage of residents maintaining stable housing over a 6-to-12-month period.
• Crisis Intervention Outcomes--Number of crises de-escalated without hospitalization or emergency service involvement.
• Daily Living Skills Improvement--Documented growth in resident independence in ADLs (Activities of Daily Living) and IADLs (Instrumental Activities of Daily Living).
• Medication Adherence Monitoring--Frequency and accuracy of support provided to ensure medication routines are followed.
• Visit Compliance Rate--Percentage of scheduled home visits completed per week/month.
• Documentation Timeliness--Progress notes and service documentation submitted within 24-48 hours of service delivery.

Requirements:

• A Master's or Bachelor's degree in a human services-related field with 2 years experience in human services; OR an Associate's degree in a human services related field and 5 years experience in human services; OR a High School diploma and 7 years experience in human services.
• Strong knowledge of mental health principles and practices.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively in a team setting.
• Proficiency in using Electronic Health Record (EHR) systems.
• Strong computer knowledge: typing, communication tools, scheduling tools and web browsers.
• Proficiency in Microsoft Office suite and database management systems.
• Must have a valid Driver's License and a driving record that is satisfactory to our insurance carrier.
• Must maintain current NYS Defensive Driving certification.
• Fluency in English (Reading, Writing, Speaking).
• Must be cleared and maintain clearance by applicable regulatory clearances as required.
• Legally eligible to work in the United States without sponsorship.

Special Considerations : 

• Position may require extended periods of sitting and computer-based work.
• Work hours, reporting structure, and on-site location are subject to change based on program needs.

The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter.
