Administer prescribed medications and monitor patient responses, reporting changes as needed. Perform and document ongoing assessments, track progress toward rehabilitation goals, and adjust plans as appropriate. Collaborate with physical, occupational, and speech therapy teams to implement and optimize the patient’s rehab plan. Monitor vitals, manage wounds, monitor for signs of complications, and coordinate with the care team for escalation as required. Ensure accurate documentation, participate in discharge planning, and educate patients and families about care plans and progress.
