Home Health Registered Nurse

Signature Healthcare at Home – Registered Nurse (Home Health) Location: Skagit County, Washington (Mount Vernon, Burlington, Sedro-Woolley, Bow, Anacortes, Hamiliton, Concrete). Shift: Monday‑Friday day shift, self‑directed scheduling based on patient needs and territory. Duties and Responsibilities Complete an initial and ongoing comprehensive assessment of patient and family/caregiver living situations to determine care needs. Provide a complete physical assessment and history of current and previous illness(es). Establish an individualized plan of care in collaboration with the certifying medical provider based on assessment and patient goals. Regularly re‑evaluate patient and family/caregiver needs and revise the plan of care as necessary. Develop interventions and goals, involving the patient and family/caregiver in the planning process. Work autonomously to initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the medical provider. Provide health‑care instructions to the patient and family/caregiver per assessment and plan. Facilitate patient self‑sufficiency or appropriate caregiving. Act as case manager when assigned, coordinating patient care for the caseload. Document patient services and status timely and accurately, meeting professional practice standards and reimbursement requirements. Communicate with the medical provider regarding patient needs, report changes in condition, and obtain/receive orders as required. Teach the patient and family/caregiver self‑care techniques as appropriate. Provide medication, diet, and other instructions ordered by the medical provider and use opportunities for health counseling. Work in concert with the interdisciplinary team, attending required staff meetings, interdisciplinary team meetings, and care coordination activities, and complete mandatory education requirements to maintain competence. Coordinate patient care in tandem with LPN/LVN as assigned by the Clinical Manager. Knowledge, Skills and Experience Graduate of an accredited school of nursing. One to two years of recent acute care experience in an institutional setting. One to two years of recent experience in home health preferred. Current Washington State Registered Nurse license. Current CPR certification. Must be licensed and able to practice in Washington State. Valid driver’s license and access to a vehicle with insurance; ability to travel for agency‑related duties. Preferred: clinicians with experience in Home Health or post‑acute care and familiarity with Homecare Homebase EMR. Schedule, Pay and Benefits Full‑time (32+ hours per week). Monday through Friday. Hourly wage $50–$55 (dependent on experience) plus mileage reimbursement. Medical, dental and vision plans available the 1st of the month after hire. Paid company holidays, 2+ weeks PTO and 401(k) match after 90 days.
