Primary Care Nurse Practitioner or Physician Assistant Location: Hartford, CT, US Experience: Entry Level Salary: $110,000 - $175,000 per year We are hiring a Primary Care Nurse Practitioner / Physician Assistant for a full-time outpatient opportunity in Hartford, CT. This is a strong role for an APP seeking a mission-driven primary care setting with a predictable weekday schedule, loan repayment eligibility, and the chance to build long-term relationships with patients. The provider will deliver comprehensive primary care services to an adult patient population while working alongside an established interdisciplinary team. What You’ll Do Provide comprehensive primary care visits, assessments, and ongoing management for adult patients Diagnose and treat acute and chronic medical conditions Order and interpret diagnostic tests and labs Prescribe medications and manage treatment plans as appropriate Educate patients on health maintenance, prevention, and chronic disease management Collaborate with physicians, nursing staff, and the broader care team to ensure continuity of care Maintain accurate, timely documentation within the EMR What We’re Looking For Active or eligible Connecticut licensure as a Nurse Practitioner or Physician Assistant Prior experience in Primary Care, Family Medicine, Internal Medicine, or a related outpatient setting preferred Strong clinical judgment, communication, and patient education skills Comfortable managing a broad adult primary care panel Mission-driven, team-oriented, and committed to quality community-based care About Us Established community health organization in Hartford, CT serving a diverse patient population through high-quality, mission-driven primary care.
