Medical Social Worker - Day ShiftCompensation: $87,000 - $131,000 annually.Make a Difference in Patient Care at this Magnet Hospital!We are seeking a dedicated and compassionate Social Work Care Manager to join our Care Management team. This full-time, day shift position is Monday-Friday, 9 am-5 pm, with rotating weekend coverage (Saturday & Sunday).About the Role:As a Social Work Care Manager, you will play a crucial role in ensuring patients receive the appropriate support services for a smooth and successful discharge. You'll conduct comprehensive assessments, identify high-risk patients, coordinate care, and facilitate transitional planning to prevent readmissions and improve patient outcomes.Responsibilities:Conduct comprehensive initial assessments upon admission, screening all patients for high-risk indicators.Provide ongoing reassessments of patient needs throughout their hospital stay.Coordinate multidisciplinary care within and between hospitals.Facilitate all transitional planning activities, including:Patient and caregiver educationArranging aftercare servicesEngaging with commercial payersAddressing root causes for high utilizersCommunicate and collaborate effectively with the healthcare team.Contribute to departmental goals of reducing length of stay, preventing readmissions, reducing denials, and improving patient satisfaction.Maintain accurate and timely documentation according to department requirements.Qualifications:Master of Social Work (MSW) degree from a CSWE-accredited School of Social Work.Licensed Social Worker (LMSW/LCSW) in New York State.Two years of acute care hospital experience or related setting preferred.NYS SCREEN Certification (must be obtained within the first 3 months of employment).
