Behavioral Health Evaluator & Assessment Specialist
Centerstone Medical Group
Adult & Child Health is hiring an Evaluator in Indianapolis. The role conducts comprehensive screenings and initial assessments across the lifespan, determines service eligibility, and develops treatment plans with provisional diagnoses. You may provide brief therapy and safety planning as needed, while ensuring documentation in the EHR and coordinating referrals.
