Physical Therapist (PT) – Home Health | Lakewood, NJ | PRN / Part-Time / Full-Time | W2 or 1099

Are you a licensed PT ready to leave behind the double-booking, productivity quotas, and clinic burnout? Stern at Home Therapy is hiring Physical Therapists at all commitment levels — PRN, part-time, and full-time — to deliver one-on-one care in patients' homes across Lakewood and Ocean County, NJ.

Our PTs set their own schedules, treat one patient at a time, and have a dedicated admin team behind them handling the logistics. Whether you want a full caseload or just a few visits a week, we've built this role to fit your life — not the other way around.

What the Job Looks Like:

You'll evaluate and treat a primarily geriatric population in their homes across Lakewood, Toms River, Brick, and surrounding Ocean County communities, creating and executing your own individualized treatment plans with full clinical autonomy. A dedicated admin team manages scheduling, documentation support, and coordination — you focus on the patient in front of you. Shadowing is available if you're newer to home health or just want to see the model before committing.

Our Stern Scheduling Assurance Plan backs up our promise to keep your caseload full: if we can't fill your schedule and can't replace a cancellation, you get paid for up to 2 cancellations per month.

Compensation

W2 Full-Time:

• $55–$75 per visit, tiered.
• Tier 1 (1–20 visits/week): base rate.
• Tier 2 (21–32 visits/week): +$5 per visit.
• Tier 3 (33–40 visits/week): +$5 per visit.
• Salary option: $92K–$110K based on experience.

1099 Full-Time:

• $70–$80 per visit, tiered.
• Tier 1 (1–20 visits/week): base rate.
• Tier 2 (21–32 visits/week): +$5 per visit.
• Tier 3 (33–40 visits/week): +$5 per visit.

W2 Part-Time:

• $55–$70 per visit.

1099 Part-Time:

• $72–$85 per visit.

PRN (1099):

• $72 per follow-up visit.
• $85 per evaluation.

Full-Time Benefits (W2):

What We're Looking For:

• Active PT license in New Jersey (required).
• Reliable transportation (required).
• Minimum 1 year of U.S. clinical experience (preferred).
• CPR certification (preferred).
• Home health experience preferred — not required.
• Strong communication and bedside manner.
